Position: Home Health Physical Therapist (PT)
Compensation: Per-visit rate up to $135 per visit
We are seeking a qualified Home Health Physical Therapist to provide comprehensive rehabilitative care to patients within their home environments. This role serves a diverse patient demographic featuring a significant Hispanic and Filipino population; therefore, professional fluency in Spanish or Tagalog is highly desirable to optimize patient communication and outcomes. Key Responsibilities:
- Conduct thorough initial evaluations, functional assessments, and home safety screenings to determine patient rehabilitative needs.
- Develop, implement, and modify individualized physical therapy plans of care in coordination with attending physicians and the interdisciplinary care team.
- Administer therapeutic interventions, including gait training, mobility enhancement, therapeutic exercise, and fall prevention strategies.
- Instruct and educate patients, family members, and caregivers on home exercise programs, safe transfer techniques, and the proper use of assistive equipment.
- Monitor, document, and maintain accurate clinical records of patient progress, treatment responses, and functional milestones in accordance with regulatory standards.
- Formulate safe discharge plans and recommend community resources or ongoing adaptive measures as necessary.
