Position: Home Health Physical Therapist (PT)
Compensation: Up to $135 per visit
Position Overview:
We are seeking a qualified Home Health Physical Therapist to deliver high-quality, patient-centered care within our service territory. The local patient demographic includes substantial Hispanic and Filipino populations; therefore, bilingual proficiency in Spanish or Tagalog is highly preferred to facilitate clear communication and culturally competent care. Key Responsibilities:
- Conduct comprehensive initial and ongoing physical therapy assessments within patients' home environments.
- Design, implement, and modify individualized plans of care in alignment with physician directives and patient goals.
- Administer functional mobility training, therapeutic exercises, gait training, and pain management interventions.
- Educate patients, families, and caregivers on home safety protocols, fall prevention, and independent home exercise programs.
- Complete accurate, timely clinical documentation and outcome assessments in compliance with agency, state, and federal regulatory standards.
- Collaborate with the interdisciplinary healthcare team, including physicians, nurses, and other therapy professionals, to ensure coordinated continuity of care.
- Re-evaluate functional status periodically and formulate appropriate discharge plans once therapeutic goals are achieved.
